DIRECTIONS:
From the East Bay: Take the 580 (Richmond Bridge) going west towards San Rafael. Then stay on the right and merge onto 101 north towards Novato. Take the Freitas Pkwy Exit. Keep right at the fork and proceed (east) toward Civic Center Dr/Frontage Rd. Turn left (north) on Civic Center Dr/Frontage Rd. Travel 0.8 mile to Mitchell Blvd. Turn right on Mitchell Blvd. We are on the left hand side of the street. Suite 10, Triad Of Health, is across from Tina’s Nails.
From San Francisco: Take the 101 north across the Golden Gate Bridge. Drive past Central San Rafael. Take the Freitas Pkwy Exit. Keep right at the fork and proceed (east) toward Civic Center Dr/Frontage Rd. Turn left (north) on Civic Center Dr/Frontage Rd. Travel 0.8 mile to Mitchell Blvd. Turn right on Mitchell Blvd. We are on the left hand side of the street. Suite 10, Triad Of Health, is across from Tina’s Nails.
From Petaluma/Santa Rosa/Mendocino: Take the 101 South. Take the Lucas Valley Rd/Smith Ranch Rd Exit. Turn right at Lucas Valley Rd/Smith Ranch Rd (east on Smith Ranch Rd). Turn right (south) on (Redwood Highway) Frontage Rd. Travel 0.3 mile. Go straight past Mitchell Blvd. Turn left into La Plaza Office Complex. If you walk in through the front where the Palm trees are then we are in the right portion of the quad. We are not on the street or parking lot side.
