New Patients

If you are ready to get started with getting rid of your symptoms with the aid of a San Rafael Functional Medicine puzzle-solving coach, please click on the New Patient Forms in the blue rectangular box below and proceed to download and print out and then handwrite out the New Patient Male Forms if you are a male and the New Patient Female Forms if you are a Female. We charge a $10 fee if you need us to provide you with the forms in the office and we will need an extra 30 minutes of time if you choose this option. Call our office to schedule your initial exam and be sure to bring your filled out forms in with you to the exam
